PNW Arthritis & Rheumatology is a new private practice committed to delivering exceptional, patient-centered care with warmth, efficiency, and clinical excellence. Founded by longtime Tri-Cities residents and seasoned rheumatology providers Jeannette Hart, PA-C, MCHS, MHA and Dr. Derek Peacock, our clinic was created to restore the essence of relationship-based medicine—where collaboration, continuity, and trust are at the heart of every patient experience.
With a focus on holistic and team-based care, we’re building a practice that blends innovative treatments with deep clinical expertise. Our mission is to make high-quality rheumatology care more accessible for patients across eastern Washington and northern Oregon—without compromising on the personal connection that makes medicine meaningful.
Jeannette Hart is a board-certified physician assistant with over 13 years of experience specializing in rheumatology. She earned her Master of Clinical Health Services through the University of Washington’s MEDEX PA Program in 2012 and later completed a Master of Health Administration to deepen her expertise in clinical operations.
Jeannette began her career in private practice and successfully led through transitions involving hospital acquisitions and system mergers—all while enhancing patient access and streamlining care delivery. She has a strong passion for mentorship, regularly precepting PA students and family medicine residents, and currently serves as the Pacific Northwest Director and faculty member for RhAPP (Rheumatology Advanced Practice Providers).
Outside of medicine, Jeannette is a proud wife, mother of two competitive dancers, and lifelong Tri-Cities resident. She enjoys swimming, entertaining friends, and spending time with her family and their dog, Sport.
Dr. Derek Peacock brings more than two decades of clinical leadership to PNW Arthritis & Rheumatology. He earned his medical degree from King’s College School of Medicine in London, completed internal medicine training in the UK, and completed his rheumatology fellowship at UCLA.
Dr. Peacock founded and led a large private rheumatology practice with an on-site infusion center and clinical research division, conducting more than a decade of clinical trials as Principal Investigator at Apex Clinical Research. A committed mentor, he helped train five advanced practice providers in rheumatology.
After a short early retirement, Dr. Peacock returns to full-time practice with renewed focus and passion. He is a devoted husband, father, and proud grandfather who continues to be inspired by curiosity, compassion, and a dedication to patient-first care.
We’re proud to offer specialized care for a wide range of autoimmune and inflammatory conditions and will soon expand into wellness and integrative services to support long-term health and recovery.
We are actively contracting with major insurance providers and will update our accepted plans regularly as agreements are finalized. A full list will be available closer to opening.
