Your First Visit

Preparing for Your Appointment

We are a referral-based clinic. After we receive a referral from your primary care or specialty provider, our team will reach out to schedule your first appointment.

To help us make the most of your time, we ask that you bring:

  • A current list of medications (including supplements)
  • Recent lab results or imaging reports, if not sent by your referring provider
  • Your insurance card and photo ID
  • Any questions or concerns you’d like to discuss

 

We’ll take time during your visit to review your health history, conduct a physical exam, and begin to build your care plan together.

Be on the look out for emails and texts allowing you provide intake information electronically.

Patient Portal Access

Stay Connected Between Visits

We use a secure electronic medical record system with a patient portal for ongoing communication and care management.

Through the portal, you can:

  • View visit summaries
  • Access lab results
  • Send and receive secure messages
  • Request medication refills

We are actively contracting with major insurance providers and will continue to update our accepted plans as agreements are finalized.
